Evaluation Materials -- Faculty

It is your responsibility to prove that you have met the requirements for the personnel action you requested.

This list includes both materials that you must include in your portfolio and materials that are available to evaluators and may be used in the evaluation process.

 

  • Your must submit request  a personnel action  to your chair by April 1 of the previous academic year.
     

  • Appendix A-1 -- your responsibility -- See Faculty Evaluation Criteria.  I can't overstress how important it is to be careful about filling out this form.
     
  • Your Updated Comprehensive Résumé -- your responsibility
     
  • Your "narrative" -- optional but recommended  -- your responsibility.  See the section on General Advice for Constructing Portfolios.
     
  • Your course documents (course outline including a syllabus and a bibliography when used) -- your responsibility
     

  • Your current transcripts -- your responsibility 
     

  • Other information or materials supplied by you -- that is, the materials you submit as evidence that you meet the criteria for the personnel action you are seeking.  -- your responsibility.  Again see General Advice for Constructing Portfolios.

  • Validated, relevant materials supplied by your chair and/or the vice president. -- Chair's or VP's responsibility
     
  • Your personnel file -- please check it.  The only valid personnel file is the one maintained in the Human Resources Office.  Departments and Department Chairpersons should not be maintaining any other files on you.  Nothing should have been placed in your official personnel file without notification to you.   You have the right to write a response to be placed in your file when something is placed there that could cause you harm.  Please consult with an MSCA officer before doing so.  If you find something that you didn't know about and  thus shouldn't be in the file or something that was not placed in your file legitimately, contact our grievance officer,  Sandy Faiman-Silva, immediately.  You may need to file a grievance to have it removed, and grievances must be filed within ten days.  
     
  • Classroom visit reports from your chair and Peer Evaluation Committee
    (Appendix D-1)  (PECs don't participate in the tenure process except at the candidate's request).  -- Chair's responsibility
     
  • Your chair's and Peer Evaluation Committee's reports (Appendix D-2)  -- Chair's responsibility 
     
  • Your responses to the various evaluations. -- Your responsibility to submit them on time; Chair's or VP's , to transmit them.  
     
  • Your Committee on Tenure's vote, minutes, letter if you are a candidate for tenure. -- VP's responsibility 
     
  • The Promotion Committee's judgments if you are a candidate for promotion.  -- VP's responsibility
     
  • Student Evaluations -- Chair's responsibility.  See General Advice for Constructing Portfolios.
  • Additional Evaluation Reports – for Alternative Professional Responsibilities -- Chair's responsibility
     
  • Vice President's Report (Appendix G )  -- VP's responsibility
     
  • Action of the President and/or Board of Trustees -- President's responsibility
